Rhinoplasty consultation: more than an hour for your nose

The initial consultation is the most important appointment of your rhinoplasty. It creates the picture we work from later in the operating theatre: we examine your nose from the outside and from the inside, simulate your possible outcome in three dimensions and tell you honestly what is achievable in your case and what is not.

We really do take the time

A rhinoplasty has to run perfectly, and the work for that begins long before the day of surgery. This is why we set aside more than an hour for you in the initial consultation. That hour is not about moving you towards an operation. It is about two people ending up with the same picture in mind: you and the surgeon who will operate on you.

This is possible because the Kuzbari Centre has its own state-of-the-art operating rooms. They are equipped precisely for rhinoplasty — and they give us the luxury of completely free scheduling. How long a procedure takes is decided here by the findings, never by the appointment book. A rhinoplasty at the Kuzbari Centre takes one to four hours; that range is deliberate. The same attitude applies to the conversation beforehand.

Whoever advises you also operates

In the initial consultation you speak to the surgeon who will later operate on you and see you again at the follow-up appointments. There is no handover to a third person, and no consultation held by anyone other than the person at the operating table.

What happens in that hour

A great deal happens in that hour, and if you are considering a rhinoplasty you should not miss it:

  1. Minutes 1 to 15Your concern

    First we listen. What troubles you about your nose, since when, and in which view do you notice it — in the mirror, in photographs, on video? That distinction matters more than it sounds: some people are troubled by a profile, others by the frontal view. Each leads to a different plan.

  2. Minutes 15 to 30The examination

    We assess the bridge, the tip, the nostrils and the columella from the outside and palpate the cartilaginous and bony framework. Then we look inside with a nasal speculum: septum, turbinates, nasal valves. The thickness of your nasal skin is established as well, because it co-determines how fine an outcome can be.

  3. Minutes 30 to 50The 3D simulation

    A three-dimensional scan of your face is created, and on that model we shape the desired changes together — in profile, from the front and from below.

  4. Minutes 50 to 60Technique, risks, limits

    We explain which technique suits your findings, what it can achieve and where its limits lie. A rhinoplasty is a procedure that carries risks; we talk about them openly, and not first on the day of surgery.

  5. To closeCost estimate and time to think

    You take home a written cost estimate covering every service included. Then you decide at your own pace. There is no deadline and no follow-up call pressing you.

The examination: from the outside and from the inside

The nose is the organ you breathe through and at the same time the most conspicuous element of your face. Altering the outer shape means working on the framework that keeps the airways open. That is why the initial consultation always covers both — shape and function — even when it is only the shape that troubles you.

From the outside we assess the proportions in relation to forehead, chin and cheeks, the line of the bridge, the projection and rotation of the tip and the position of the nostrils. We palpate how strong your cartilage is, because that determines how much stability the framework brings of its own accord.

From the inside we check whether the septum stands straight, whether the turbinates are enlarged and whether the nasal valves collapse on inhaling. Many people learn in that minute why they breathe worse at night than during the day. Where such a cause exists, it can be treated in the same operation.

Skin thickness has a say

Thin nasal skin follows every fine contour — which is exactly why it also reveals the smallest irregularity. Thicker skin is less forgiving of detail and takes longer before the refined shape appears. We establish your skin thickness in the initial consultation and tell you what it means for your outcome.

How thick is your skin?

The 3D simulation: three perspectives instead of one

State-of-the-art 3D technology lets the desired nasal shape be viewed in advance. Individual surgical steps are demonstrated visually. That way we can respond to your wishes and define a realistic treatment goal together with you.

Our 3D simulation shows your future nose not only in profile, but also from the front and from below. You therefore see your possible outcome from exactly the angles from which other people see you every day — and we discuss the same image before surgery.

That is more than a technical detail. A „narrower nose“ means something different to everyone. On the three-dimensional model a word turns into an image that can be discussed: half a millimetre less of a hump here, a little more rotation of the tip there — and you see immediately what it does to your face.

Which technique suits your nose

At the end of the initial consultation you know not only what can be changed, but how. We work a great deal with the modern nose-bridge-preserving technique known as dorsal preservation, as well as with hybrid techniques — not exclusively with traditional structural septorhinoplasty. The natural bridge of the nose is preserved rather than taken down and rebuilt.

This has three advantages: the outcome looks particularly natural, it is stable in the long term, and the risk of later irregularities along the bridge goes down — which counts above all with thin skin, where every tiny edge becomes visible. Certain deviations also recur less often. Which route is the gentler one is decided by the findings: the nose-bridge-preserving technique is not the right choice for every nose, but for many findings it is the finer one.

We stabilise the tip of the nose using modern cartilage-supported techniques. This considerably reduces the risk of drooping of the nasal tip over the years. That too belongs in the initial consultation, because it helps decide how your nose will look in ten years — not only in ten months.

When the nose has been operated on before

Four in ten nasal operations at the Kuzbari Centre are revision rhinoplasties on a nose that has already been operated on. Such procedures call for particular experience: the tissue is scarred, cartilage is sometimes missing, and the starting point differs from person to person. We deliberately take such cases on.

In these cases the initial consultation often takes longer, and it is more honest than comfortable: not every goal is reachable on a previously operated nose. What is reachable we tell you in the conversation — with the simulation in front of us and, where available, your surgical report in hand.

If you decide to go ahead, preparation follows: the informed-consent discussion, medical history and laboratory values, anaesthetic preparation through the pre-anaesthesia clinic in the building and the question of which medicines have to be stopped beforehand. Yes. We pick out cases whose starting point resembles yours. Every nose heals differently, so an outcome shown to you cannot be transferred to you — but it makes the order of magnitude of what is possible visible.

Will I learn in the initial consultation what my rhinoplasty will cost?

Yes. After the examination you receive a written cost estimate covering every service — from the operation through anaesthesia and the overnight stay to all follow-up appointments. A first-time procedure starts at 9,350 euros.

Can I choose who advises me?

Is breathing examined in the initial consultation as well?

Always, even when it is only the shape that troubles you. We check the septum, the turbinates and the nasal valves. Where a cause of obstructed nasal breathing is present, it can usually be treated in the same operation.

What happens if I decide against surgery?

Then the matter is closed as far as we are concerned. You can come back at any time; many people get in touch again months later. No call will follow to press you.
